| Satisfaction Guarantee Policy |
| If not satisfied with a class or unable to participate for
any reason, please contact us immediately. Our policy is to provide a 100%
refund to anyone who makes a written request before the start of the second
class.* Requests received after the second class will entitle participants to a
prorated refund. Prorated refunds will be based on the number of classes
remaining at the time the application for refund is made. *Please note!
Satisfaction guarantee does not apply to adult sports leagues, Drivers Education classes, one-time
classes, special events or trips. There are no refunds on class materials
purchased. Refunds can not be requested online.

| Accident Insurance |
MCS does not provide accident insurance for participants
in any of its programs/activities. Participants assume all inherent risk of
injury resulting from their involvement in programs or activities pursuant to
this application.
Photographs
Participants, upon payment of activity/program fee, give consent to the use of
any photographs taken by the MCS office, its employees, agents and elected
officials and agree such photographs shall be the property of the MCS
Department. Participants are not entitled to compensation of any kind for the
use of such photographs.
